Please find listed a number of Self-Assessment forms for use by Patients. These have been created for patients so as to avoid the need to make an unnecessary appointment with a Clinician. Please note that the use of these forms is in no way intended to replace the need for you to attend for regular health management appointments.

If you have one of the following conditions please use the appropriate Self Assessment form:

	Epilepsy,
	Asthma,
	High Blood Pressure. (BP Readings). BP Monitors can be bought from the Practice or your GP can advise you as to the best model/type for you to use.

It should be noted that these forms are e-mailed to the Practice and are not encrypted. If you wish to use secure messaging please register for the online services.
